When a person with disabilities turns 18, they may be eligible to receive services through long-term care programs. Long-term care programs can help with funding for medical needs, day service options, or respite care, to name a few. They also help families to navigate the "adult world" for people with disabilities. When you meet with an Information and Assistance Specialist at the Aging and Disabilities Resource Center (A.D.R.C.), they will help to explain these enrollment options and what services and options your young adult qualifies for.
In Marathon County, you have three options for long-term care programs. You can switch to a different provider anytime, so you don't need to worry about choosing the "wrong one."
1. Family Care

2. The IRIS Program
The IRIS Program is a Medicaid Home and Community-Based Services (H.C.B.S.) waiver for self-directed long-term support. A.I.R.I.S. is available to Wisconsin residents determined financially eligible for Medicaid, functionally in need of a nursing home or Intermediate Care Facility for Individuals with Intellectual Disabilities (I.C.F./I.I.D.) level of care, and living in a county where managed long-term care and IRIS are available. A program is an option for adults with long-term care needs. 3. Lakeland Care
Lakeland Care was one of the first Managed Care Organizations to provide Family Care benefits in the state more than 16 years ago. We can help meet your long-term care needs. Our Family Care program is designed to help eligible elders and adults with disabilities live independently – at home and in their community. Members partner with our care teams and local providers to get their needed support and services. If you are a senior in high school, an adult with a disability, or a concerned family member or friend, you may seek support in meeting your long-term care needs.
